Through programs like Home Care Services and Mid-Columbia Meals on Wheels, we provide essential support to seniors, ensuring they maintain independence and well-being.

The Service Coordinator is responsible for evaluating and determining client eligibility for home-delivered meal services. This role involves conducting assessments, maintaining accurate documentation, and coordinating meal services in compliance with state, federal, and agency guidelines. The Service Coordinator will interact directly with clients, respond to referrals, perform home visits, and update client information regularly. Additionally, they will serve as a liaison between clients and community resources, ensuring timely service delivery while maintaining confidentiality and professionalism.


Key Responsibilities

Client Intake & Assessment

  • Respond to referrals and conduct timely phone intakes and home assessments.
  • Determine client eligibility for home-delivered meal services in line with state, federal, and agency standards.
  • Maintain accurate assessments, case notes, and eligibility documentation in the CARE eVantage database.

Service Coordination & Support

  • Arrange meal services for eligible clients and assign them to delivery routes.
  • Monitor Community Options Program Entry System (COPES) authorizations and coordinate with Aging & Long-Term Care (ALTC) case managers.
  • Track new sign-ups, service terminations, and denials; provide monthly reports to the Nutrition Program Director.
  • Maintain communication with clients, meal-delivery staff, and community partners to ensure continuity of care.

Community Resources & Outreach

  • Provide clients with information and referrals to community resources.
  • Support and promote the Essential Meal Program while addressing service concerns.
  • Cross-train in other Meals on Wheels roles, including Site Assistant, Transporter, and delivery, as needed.

Administrative Duties

  • Uphold confidentiality, professionalism, and agency standards in all client interactions.
  • Assist with meetings, training sessions, and special projects as assigned.

Skills & Qualifications

  • Strong communication and interpersonal skills for client interaction and collaboration.
  • Ability to work independently, prioritize tasks, and exercise sound judgment.
  • Detail-oriented with accurate record-keeping and compliance awareness.
  • Proficiency in data entry, office equipment, and computer applications.
  • Associate’s degree or equivalent experience; 2+ years in customer service or human services preferred.
  • 6+ months of office experience; bilingual (English/Spanish) preferred.
  • Ability to lift up to 25 lbs and perform home visits safely.

Working Conditions

  • Frequent driving for in-home assessments; primarily office-based with occasional remote work.
  • Regular sitting, standing, bending, and light lifting up to 25 lbs.
  • Requires professionalism, confidentiality, and flexibility to meet client needs.

Contingency of Employment

  • Acceptable background check and driving record.
  • Valid Washington State driver’s license, insured personal vehicle, and proof of insurance.
  • Compliance with Chapter 246-215 WAC Subpart B “Employee Health.”
